8/23/2023 EnchantaVaughn Approved Behavior: Upon arrival to the home setting, client appeared to be in a positive mood. QMHP-C engaged mother in a discussion to determine behaviors since the last session. Mother verbalized to QMHP-C she has trouble with getting client to talk about her feelings as she is very quiet reverts to aggression when angry. Behavior: Client expressed resentment ( which results in aggression ) for not being able to be open with her mom during this difficult moment in time. Client continued to show guarded and distant behaviors. Client admitted that some of her decisions can cause her to hurt other people close to her. Intervention: QMHP inquired about clientâs day in the home and what things sheâs doing to make it a good day. QMHP educated client about what she can do to make her daily activities more fun and interesting, such as reading her favorite book, drawing in a journal, stretching and closing her eyes and practicing the mindful behaviors learned in sessions. QMHP offered active listening skills when client wanted to speak. QMHP provided client with encouragement to demonstrate the positive behaviors and expectations they had been talking about. QMHP continued to assist client with managing symptoms of ADHD by reviewing and role-playing effective coping skills. QMHP monitored effects of meditation and coping practices and documented the progress in her treatment chart. Intervention: QMHP led the client through each therapeutic day treatment exercise. QMHP motivated client to respond to exercise and express her feelings. QMHP modeled proper positive behavioral expectations for the client to encourage additional layers of support and understanding of rules and expectations. QMHP processed with client on what she needs to do to achieve ISP objectives. QMHP encouraged client to be the example for others. QMHP encouraged client not to follow others she sees that are doing wrong. QMHP commended client on her efforts to maintain her ISP objectives as best as she could even with minor difficulties. Response: Client displays normal behavior in therapeutic setting. Client exhibited a positive attitude toward mom and counselor at the beginning of the session. Client has implemented the correct behaviors in session toward in-home staff. Client was observed avoiding disruptive and impulsive actions demonstrated when client proceeded to walk away and utilize deep breathing. Client was observed intervening and regulating her emotions. Client knows what is expected of her and is trying to make better choices moving forward. Response: Client was observed in her seat and raising her hand when she wanted to ask a question. Client interacted well in the session and demonstrates good work ethic. Client finds more excitement in learning about her mental illness. Client was fully engaged in the activities presented to her today, showing great sportsmanship and responsibilities in the home.
